Membership Eligibility

Am I Eligible for Membership? 

Our field of membership includes current and retired MTA and Northwest Hospital Employees including their families and household members. We also serve employees of our affiliated businesses.

●Employees of Our Affiliated Businesses 

Contact us to find out if your company is already an Affiliated Business, or if they are eligible to join.

To Become a Member, You’ll Need:

  • An opening deposit to fund your membership (provided prior to or at opening)
  • U.S. Social Security Number
  • An unexpired US Government issued ID (State issued Identification Card, State issued Driver’s License, or US Passport). Proof of Address will be required if current address does not match.
  • Signed Family Member Statement (If eligible by immediate family member).
